Don’t Just Do Something, Stand There: Pressing Pause on Acetaminophen Use in Infants and Children | American Journal of Respiratory Cell and Molecular Biology First synthesized by Morse in 1887, acetaminophen (APAP) did not gain popularity as an analgesic agent until the 1970s (1). Having gained a position as a β€œhousehold” analgetic and antipyretic agent, its use has spread to infants and children and has been considered safe for use in pregnancy. Nonetheless, with randomized controlled trials involving pregnant women participants being ethically impermissible when there is equipoise about safety, much of our understanding about drug safety in pregnancy relies on animal models and accidental exposure in humans. Historically, a lack of gross malformation of organs and limbs served as a surrogate endpoint for safety; however, recent preclinical data and clinical studies have raised concern that exposure to APAP during pregnancy and early childhood is associated with abnormal brain and lung development, including the association of APAP exposure with increased risks of autism, attention deficit disorder, and asthma (2–5).
