If I'm injecting something that can be sore (like propofol), I'll say it might feel strange/tingly/weird, but it'll be short-lived
I just say what I'm going to do; "small injection of local anaesthetic now". That way the patient can decide how much it hurts (or not), rather than me giving them a pain prompt

12.07.2025 16:14 β π 1 π 0 π¬ 1 π 0I used to, then I moved to placing amps & labelled syringes in a tray, ready to quickly draw if needed.
My place stocks prefilled atropine, ephedrine, metaraminol. They go into a tray, add a glyco amp and a syringe.... (& check there's sux & in the fridge).
The move to prefilled was environmental
I have recently refused to work until appropriate sized scrubs found.
I got sick of exposing myself when having to wear over large sizes. Particularly the elasticated trousers that were falling down constantly.
(Admittedly, I sat in my outdoor clothes in the coffee room to stage my protest)
Untucked- otherwise I can't use the pockets on the top.
(Although, secretly, as I really get cold, I often have a tucked in top on underneath to try and keep warm!)
Leakage was more like 95% with us. Manifolds now decommissioned at most of the hospitals in our health board. Portable cylinders available to plug into the back of the machine if needed

(I still visit there for a list once a month - block list of course!)
I've seen some of @athmathottungal.bsky.social work where she's using for neuromodulation.... very clever stuff.
One day I'll remember to pin her down on the specifics and give this largely ignored bit of kit a new lease of life
100% sterile gel if you're planning on puncturing the skin...
(Agreed that sterile water/saline will also work as acoustic coupling agent!)

My kids are bilingual and speak English & Cymraeg fluently!
There are a load of regional accents yn Gymru, much like in other parts of the world....

Will block kids asleep as routine.
Probs, in my hands as experienced blocker, risk of neural injury is v low awake or asleep.
In a less experienced pair of hands, the confidence of no paresthesia/pain during block is useful!

It's not really a thing where I work.
We still can use epidurals on our wards, so lots of cases get that. Most general & urology surgeons put in rectus sheath catheters if epidural not used.
