I came across this β€œpractical guideline” for major heritable thrombophilia. I am not sure I agree with some of their recommendations - should generate an interesting debate though. Also have never tested for the F2 AT resistance mutations!

Population-Scale Studies of Protein S Abnormalities and Thrombosis This cross-sectional study evaluates the risk associated with protein S deficiency across multiple arterial thrombosis phenotypes in population-level cohorts.

Fascinating paper; which reinforces my own clinical experience that the majority of clinically encountered type I protein S deficiency behave like a low risk thrombophilia, presumably as they have the missense variant. We need more studies like this!

I think you mean ECHELON-3.

CD30 expression is presumably a pre-requisite and it’s not common (I think only 20% of so of DLBCL have it, more common in PMBCL and plasmablastic lymphoma)

Glofit would be a better option but I suppose if you’re refractory to that, and lonc would be another option.

2) At least a theoretical risk of hyperviscosity (both IV immunoglobulins, so giving them together can make your blood too thick, leading to headaches, blurry vision, trouble breathing and risk of blood clots)

If you need both, they are usually given sequentially. ie one after the other.
