Essentially If it gets too hard to “start from scratch” use something to start with, like let’s say you just draw a guy in a suit, but then you can go “well why is he in the suit?” Or “maybe I wanna show off his pecs a bit so maybe it’s a hot day” me boom scenario!

It’s okay dude don’t be too hard on yourself, everyone struggles in their own way, I myself struggle a lot with coming up with scenarios, in my case I look at a bunch of references and see if something sparks an idea, if not I simply find a reference I like and go “what character fits here”
