You can self-confirm if you are moderately or severely immunocompromised. This means you do NOT need documentation to receive a COVID-19 vaccination you are eligible for.

Note that there's language from June on the CDC website that you can self-confirm you are immunocompromised in order to get vaccinated.

However, in real world practice, this depends on the pharmacy.
