These benchmarks represent an upward shift of moderate and severity score ranges compared to the Y-BOCS. A lack of participants in the CGI category “among the most extremely ill” precluded our ability to provide more nuance in the “severe” range (eg, “extreme/debilitating OCD”) /5

Total Y-BOCS-II scores were compared against Clinical Global Impression-Severity scores (1-7). Suggested benchmarks were: 0-14 (non/subclinical), 15-21 (mild), 22-34 (moderate), and 35-50 (severe). These were similar across gender and age groups /4

We used a sample of 3,061 participants collected from 2 samples: the LATINO study, representing Latin American/Hispanic children and adults (7-89 years) from 12 countries and a holdout sample of 78 children and adults (15-78 years) from outpatient psychiatry clinics in China /3

Benchmarks for the Y-BOCS were recently updated (Cervin at el 2022) yet benchmarks had yet to be established for the Y-BOCS-II which has, among other changes, a higher total score severity ceiling (50 instead of 40) to better capture the most debilitating OCD cases. /2

Overcorrection exposures were rated higher in one domain: anticipated peak anxiety. Findings support that adults with these themes did not find a “Fear Factor” approach the best for their identity themes, and instead preferred an approach better aligned with values and needs /6

There was a large effect overall for preference of JB-ERP over overcorrection hierarchies, including greater willingness to try exposures, greater perceived effectiveness, greater relevance, and perceiving as less derogatory or disrespectful to the target identity /5

Using a subset of data from the National OCD Survey, we asked 450 adults with identity-related OCD themes (sexual orientation, gender, racism, SES, disability status, aging) to review two symptom-specific exposure hierarchies (overcorrection vs JB-ERP) and rate them /4

Justice-based ERP does so while also considering the larger impact of our work and is better aligned with our professional ethics. For the first time, I can now offer some empirical support for this approach… /3

I have argued that some overcorrection applications of ERP have strayed from the core tenets underlying effective ERP, and justice-based ERP realigns this work through a deeper focus on core fears and uncertainty instead of catastrophizing surface level fears at face value /2
