This is my go-to strategy for procedural sedation in sick, non-intubated patients.

Start with a mildly obtunding dose of midazolam, then add a moderate dose of ketamine.

I verify orders on serotonergic agents and linezolid with patients all the time. I don't routinely make the recommendation to hold, but I am not going to complain if it is either.
