There's a monitor between the two men with a spine behind it. The images on it are of a man's skull - presumably Spock's. The next image is the doctor speaking quietly. We don't know what he's saying, it's all medical jargon not intended for a layman. He isn't speaking to Spock. He appears calm, duty-bound. There is a window behind the doctor but a light shining on his hand and Spock's ass implies that there's something offscreen. Another window, perhaps? A recording device?
