yes, that would be a concern (but also don't want to be a white saviour, sprinkling one-off largesse). Very challenging to do ethical visits to poorer countries or countires with massive inequalities, which benefit the right people.

I did do a visit to a charity which looks after children living by the railways, offering education and housing, some years ago. It did not feel too much like poverty tourism and kids seemed genuinely pleased to meet us. (Also charity has kept in touch and invites me to events). But agree often bad.

We are just rewriting our surgery timetable, cancelling GP F2F appts so we can process all the requests that come in online - because our contract now demands we keep online access open all of core hours.
This is silly and inefficient and means we will offer a worse service to pts who needs us most.

it's an intriguing idea but why not just combine tax and NI into one payment levied on all income? No one is quite sure what NI pays for nowadays and could also address some of the cliff edges within the system by introduci8ng graduated increases as income rises.

I asked an ICB which had refused to release info under the FoI but did not appear to have applied the public interest test for evidence of when this test was applied. This is what they said. There is, basically, no evidence. @mrosenbaum.bsky.social

But his CV said he was only there 1993-4 which would not be long enough for even the first part of a medical degree in Oxford. Surprised that was not picked up at the time.
