The research driving those gains is now threatened, despite the fact that HIV research has had far ranging impacts on numerous fields byond HIV, from immunology and virology, to clinical medicine and public health, to regulatory policy and practice. 🦠 🔬 🏥

📣 Over the past 40+ years, the HIV/AIDS community - led by people living with HIV, their families, friends, and loved ones, and supported by clinicians, researchers, funders, and policymakers - has saved literally millions of lives, a modern miracle of medicine, public health, and advocacy.

Therefore, together with communities around the world, clinicians, public health practitioners, and policymakers must do more, including

➡️ Educating people about TB symptoms & diagnosis
➡️ Raising awareness treatment is usually curative & free in most places once accessed

Yet with 1/4 of the world's rapidly growing population TB infected - over 2 billion people - the absolute number of people who will progress from infection to active TB disease every year is huge and continues to grow.

On World TB Day 2025, that's an unhappy 😒 fact that we must also face.

📣 Against current uncertainties for TB control and elimination, the global community 🌍 can look back on 2 decades of remarkable progress in the fight against TB 🏥, as TB incidence rates have fallen by 25% and TB death rates by 50%. The millions of lives saved is truly something to be happy 😃 about!

5/ These findings provide a roadmap for future research to adapt bCPAP interventions and design context-sensitive implementation strategies. Our goal? Ensure bCPAP is used safely and effectively to save lives, no matter the context.🌍

4/ Using a realist, theory-driven approach, we developed a program theory identifying 18 contextual factors and mechanisms across 5 levels: 1️⃣ bCPAP device 2️⃣ Local partnerships & infrastructure 3️⃣ Clinical & technical teams 4️⃣ Caregivers & community 5️⃣ Institutional practices

3/ Our review sought to understand how implementation context influences bCPAP outcomes. Past reviews suggested context-dependence but lacked a theory-driven framework.

2/ Previous studies on bCPAP for children with respiratory distress showed mixed results. While it reduced mortality in some cases, a major trial highlighted increased mortality for kids with severe pneumonia. Why the disparity? Context matters.
