β€œI want a REMS. We already have a REMS in this country for isotretinoin, an acne medication with the same level of teratogenicity. So why are we doing it for one and not the other?”

β€œI’m not saying that clinicians should never use it, but we need to be mindful of patients’ reproductive potential and educate them about the importance of contraception,” said Mariella Suleiman, M.D.

Demanding a REMS to Prevent Fetal Exposure to Valproate | Psychiatric News PsychiatryOnline.org is the platform for all American Psychiatric Association Publishing journals, DSM, and bestselling textbooks, as well as APA Practice Guidelines, and continuing medical education.

My colleague, Linda Richmond, dove into the movement within psychiatry to institute a federal safety program (a REMS) for valproate - the same type of program that is used for Accutane, the acne medication (millennials remember).

Recommendations for the Deprescribing of Psychotropic Medications This consensus statement evaluates clinical situations that would favor deprescription of psychotropic medications and provides broad consensus on its risks and benefits.

In the American Society of Clinical Psychopharmacology's recent recommendations for deprescribing psychotropic medications, a shocking 27% disagreed with the routine deprescribing of valproate in women of childbearing age.

Psychiatric News recently covered this. 1/3

How Conversion Therapy Exploits Parents’ Fears | Psychiatric News PsychiatryOnline.org is the platform for all American Psychiatric Association Publishing journals, DSM, and bestselling textbooks, as well as APA Practice Guidelines, and continuing medical education.

I wrote a three-part series exploring conversion therapy--what it's like to survive it, what it's like to lose a child to it, and what the Supreme Court's case could mean. This is the third and, perhaps, my favorite installment:

The Moral Injury of Denying Gender-Affirming Care This essay describes the author’s response to legislation on gender-affirming care.

The study found that most breast reduction surgeries among minors were performed on cisgender males.

This makes me wonder what the issue is really about - are we really worried about youth getting surgeries? Seems the concerns are misplaced, if so.

Let's add context instead of using raw numbers. A 2024 study found that, in 2019, the rate of youth who had gender-affirming surgery was 2.1 per 100,000 minors aged 15 to 17; 0.1 per 100,000 minors aged 13 to 14; and 0 among minors aged 12 or younger. jamanetwork.com/journals/jam...

The author takes issue with my claim in the story that β€œsurgeries are very rare among youth," claiming the reader should be the judge. The letter cites a 2023 study that 3,678 minors (aged 12 to 18) had gender-affirming care surgeries between 2016 and 2020.
