Wow, I hadn't considered how useful the paper boat metaphor could be in paeds practice other than for helping children to understand the frailty of their elders.
But yes, also, to helping families understand the precarious & precious good days of a child with huge health vulnerability.
Thank you.

I love this metaphor and use it often. Thank you for it x

I bet your calm, affirming responses helped her to experience those 'benign' delusions: had she been fearful, they may have felt scary instead.
My relative reinterpreted his care home as a rare bird sanctuary that he was very fortunate to be visiting. It was a longtime dream destination.

Perfect response.
Humans fear insanity and our minds deny the possibility that our current understanding is wrong. Confrontation becomes terrifying. Assurance of safety & support is essential.

Dying people frequently search for travel documents, suitcases or lists: their sense a journey has begun.

This.
A 13min mobility and strength improvement routine to learn, and use 3x weekly.
No kit.
No complicated instructions.
You can do this in your pyjamas.
You work at your own level - and watch yourself progress.

Also reminds me that, when I was part of a deaf choir (ASL, as a medical student many years ago) I was deeply moved by the transition of signs during a church ceremony from 'conversational' to 'ceremonial' to 'sung.' Getting more dance-like with each layer. Utterly beautiful to watch or participate.

Those are very clear gestures, aren't they? Actually calling upon a whole congregation to witness & understand his intention: whereas without a BSL interpreter, the non-deaf witnesses would not get those nuances watching BSL. Truly inclusive communication by gestures.

My book Bye Bye I Love You was published one year ago today! It got some very nice reviews. I talked about it a lot and had some profound interactions around it, all of which I'm grateful, so grateful, for. Thanks to @mitpress.bsky.social and my editor @philiplaughlin.bsky.social for the support.

Proud to say I know @drjoecosgrove.bsky.social and these are excellent guidelines.

Yes, closing that loop of sense-making is essential for eventual peace of mind: we can't make the unwelcome news 'good' but we can help patients & families to understand it.

Sorry @shaiuk.bsky.social, I didn't mean to shout...

But as we withdraw or recognise the limits of treatment, the intensity of caring should rise.
#IntensiveCaring.
In all settings.
And I know you achieve this so well in ICU end of life care, so that needs recognition, too.

Absolutely agree.
No-one with a LTC should present in a foreseeable crisis without a plan for action. Useful to guide patient & family, carers, first responders, ED etc, & plans made collaboratively respect the patient's values.
