Serious Sam 3: BFE's 'Technopolyp' enemy: 
Some kind of giant space octopus creature fused with an AH-64 Apache. Several large sucker-coated tentacles flail to the front and sides of the aircraft, while the cockpit is entirely covered by two bluish-purple eyes.

Somewhere in the lore it's mentioned these things fuse with the machinery deep enough that they can regenerate the mechanical parts if damaged, so I'm willing to bet this thing is mostly using the TADS/PNVS to see, rather than its biological eyes.

The Apache model used here definitely isn't 100% accurate, but I'm willing to give some leeway to an arcade shooter from 2011.

still thinking about how brave it was for serious sam 3 to give us a living helicopter tentacle monster enemy. i feel like people don't appreciate that fact enough

It's a roughly conical shape (as the context suggests), but it has several pronounced ridges near its base. It's also rather spikey in places, so surely one would need to smooth out the shape quite a bit if they intended to use it safely. Then again, when has that ever stopped anyone?
