The BICAR-ICU-2 trial just dropped!

RCT involving IV bicarb in metabolic acidosis among patients with mod-severe AKI

Bicarb didn't affect mortality but it reduced the need for dialysis & risk of bacteremia

VERY similar conclusions to BICAR-ICU-1

Caveat: some meds are designed with certain pharmacokinetic goals in mind. Aminoglycosides, for example generally work better with higher peak concentrations, so splitting the dose up is counterproductive despite achieving a smoother concentration

So I believe this is saying that the benefit of the flozin is when you stay on it. In the post trial groups the % of people in the original empa group still on empa dropped to about half…and many pts in the placebo group were started on empa, so the HR was 1
