One of the things the DoD does very well is realtime telemedicine consultation. The number of times the DoD ADVISOR line has saved my bacon in the middle of nowhere is substantial. Plus we have access to a lot of enlisted physician extenders like IDMTs or ADCs.

Telemedicine consultation can be very helpful to fill these roles. In rural Nebraska, especially during COVID, a lot of our subspecialists (like Endo) had telemed clinics and were available for phone calls from family med docs. Very helpful. One of the tough parts is the reimbursement structure.

Should the discussion be that the presumption of legality no longer applies? Or reassurance that the officer will not pass along an unlawful order? Or a review of how a service member can properly request legal advice/clarification of an order? Or “hey, know where your lines in the sand are?”

Honest question, what kind of frank discussion are you implying one should be having? Especially in the setting of “Dammit Jim, I’m a doctor, not a lawyer” and the law holding that an order presumed lawful and the accused bears the presumption of proving otherwise?
