I think there are lots of opportunities for physios to incorporate more CBT strategies into treatment with patients. And remember it’s always going to be a spectrum of how far you push into those principles. Not everyone needs as much support #physio #physicaltherapy

Even exercise has lots of context effects based on someone's and your own expectations of what will happen
This doesn't mean nothing works btw. We just have to be honest about how things work and realize how our narratives impact people long term

It leads to a spiraling effect for that person
When people get better from it's also likely even if they're really strong you just introduced novel exercise for them or they just experienced an increase in fitness level just from doing anything at all

When actually it wasn't the intervention maybe that helped
Further this is mechanistic thinking that when perpetuates ideas of human frailty and being weak causes pain. So then what happens if they're stronger and still in pain?

Then likely you see those things improve with natural history in a lot of cases leading to the perpetuation that the person needed that type of exercise

Lots of issues with the idea of core stability. One of the things is that muscle timing change and co contraction increase with lower back pain, this then leads to the "tests" used to detect core stability being positive.

While there is a small amount of data about this, there does seem to be benefits. If someone can train at longer lengths it seems to improve growth, and strength #fitness #physicaltherapy #physio #physiotherapy #health #wellness

Not exactly a novel idea but why aren't we in rehab paying more attention to the use of exercises at longer muscle lengths? #physio #fitness #physiotherapy #physicaltherapy #medicine

Always weird to see people with extremity issues in the clinic and a therapist hands them the same weight for both sides. #physicaltherapy #physiotherapy

It's always hard when our medical system lacks cohesive messaging and understanding of persistent pain issues.

Interesting how my thoughts surrounding PNE have changed since being introduced to the concept. Now it's just a part of normal conversation and natural education versus
"intervention" but seems many still think of it as a treatment

Interesting how if you think about many messages we give to patients how they don't line up with each other. Which for some people can lead to confusion if they stop and think about your recommendations #physicaltherapy #physio #physiotherapy #medicine #healthcare #pain

But my job is literally to understand how research is done, if it’s good or bad, how to vet sources, and what the information means.

Funny when people say do your own research

Like I havent made a career out of understanding research in the medical field and conveying that information in a practical way specifically to health care workers. I have dabbled in doing unique research myself.

Just a thought but it would be nice if a study could at least include their intervention descriptions in an appendix so l don't have to read 1-2 other studies to understand what you did. Or somewhere describe what was said to participants and how it was done #medicine #physio #physicaltherapy

A series of small increments in physical activity, each followed by a period of adaptation, is associated with lower rates of musculoskeletal injuries than is an abrupt increase to the same final level."
