Interesting case, indeed. PR3 in the context of C3G rather uncommon. Undetectable C3 would prompt alternative pathway investigations; also in the consideration cryoglobulins and cryofibrinogen, although C4 reduction would be more common

Why is rituximab not an option?
It is the most effective treatment option.
I guess there is no experience combining TAC and AVA so far. But I would contact the company (Amgen/CSL Vifor)

Phenotypic differences between AAV patients in Japan (compared to USA/EU) are obvious. In this study, we surveyed patients presenting during the early phase of the pandemic.

A striking difference in the use of glucocorticoids at 6 months has also been observed!
