The man is also holding a weapon - a sword, that's cutting up into the humanoid's gut and causing her intestines to spill out onto his lap. The humanoid is smiling, crying onto the man's face viscous tears and coughing blood onto him.
The man is gritting his teeth. He is covered in bandages that have been partially ripped away. The bandages around his face obscure his vision, and hair that looks like oozing blood is pouring through the gaps.
