The present participle verb starts before the simple past one. Se cayó corriendo means the same as se cayó cuando corría. The comma solution would not work because of the meaning of the verbs involved.

Most native speakers without a degree in linguistics couldn´t explain these variations. Many might not bat an eyelid at the comical example, because as speakers we make allowances to create meaning. This is how metaphors work.
As for the comical example, I´d tell my students to avoid the structure.

Se cayó rompiéndose la pierna: he was breaking his leg when he fell. Swap rompiéndose with corriendo and it is clearer: the action of correr was ongoing, interrupted by se cayó. I've read about the European alternatives. I have also read that because they train differently they feel differently. I have not used any a lot, so I cannot compare. I suppose better to start ethical then and not miss the "piracy" of the wider training.
