Last #NephJC we discussed HiLo: Hi vs Lo phos targets in HD patients #TenPostNephJC
Can restricted diets & loading patients full of phos binders be filed under βThings We Do For No Reason,β or do we finally have proof these measures save lives?

Could interleukin-6 have the potential to lead to treatments for calciphylaxis? The rare and life-threatening complication takes center stage in a new study described by Aarushi Varshney and Matthew Sparks. kidney.pub/KN1707-20 #ASNKidneyNews

Studies are hard. If they were easyβ¦weβd already have all the answers #NephJC
06.08.2025 02:06 β π 4 π 1 π¬ 1 π 0#NephJC Iβm (overly) critical, but I appreciate any and all authors insight into the challenges of study design and evaluation π»
06.08.2025 02:05 β π 5 π 0 π¬ 0 π 0I donβt feel like Iβve changed my goals, but I probably have limited the upper amount of binders I prescribe since starting practice (due to patient reluctance) #NephJC
06.08.2025 01:55 β π 3 π 0 π¬ 1 π 0Agreed, again Iβm impressed they went to publication rather than abandon ship #NephJC
06.08.2025 01:54 β π 5 π 0 π¬ 2 π 0So after another βmehβ phos trial, will we ever get the answer? #NephJC
06.08.2025 01:51 β π 1 π 0 π¬ 0 π 0Lots of limitations (so did we learn anything?) #NephJC
06.08.2025 01:48 β π 2 π 0 π¬ 0 π 0But is this study really the answers youβre looking for ? #NephJC
06.08.2025 01:46 β π 2 π 0 π¬ 0 π 0Just tell the Hi group: cheese, milk and Coke β¦ #NephJC
06.08.2025 01:43 β π 4 π 0 π¬ 1 π 0So this part of the design I like. Outcomes over numbers #NephJC
06.08.2025 01:41 β π 2 π 0 π¬ 0 π 0Surprising. I want to see more βwinβ ratios too (maybe itβs the gambler in me). #NephJC
06.08.2025 01:40 β π 1 π 0 π¬ 0 π 0I mean if you stopped all binders in Hi group and continued/added in Lo groupβ¦I feel like 1 mg/dl separation is feasible #NephJC
06.08.2025 01:38 β π 4 π 0 π¬ 1 π 0#NephJC Iβm confused why they continued knowing that the result would be significantly underpowered
06.08.2025 01:36 β π 1 π 0 π¬ 0 π 0It is not a design flaw per se. But the outcomes of deaths and admissions are so facility dependent that one would need a huge number of dialysis units, some sort of adjustment for baseline *facility* covariates and hierarchical models to estimate treatment effects with precision.
#nephjc
Seems easy in theory to get good separation given our current medical treatments #NephJC
06.08.2025 01:33 β π 1 π 0 π¬ 0 π 0Yes, the treatment of hyperphosphatemia is without a doubt worse than the disease (and never ending) #NephJC
