It's so unusual that most physicians in the U.S. have NEVER seen a case!

Agreed itβs just a question of when. Do it too early and you and I will be back in 3-4 days for the same reason.

Thank you to @sccmcriticalcare.bsky.social president for discussing climate change, gun violence prevention, threats to research and Medicaid
There is so much to do, and SCCM could be a powerful advocate
I think we agree, but you also highlight the many questions and counterpoints I see in transudative pleural effusions. Always good to discuss.
23.02.2025 21:18 β π 0 π 0 π¬ 0 π 0Also, patients without pulmonary edema present with unilateral opacification of the hemithorax without significant hypoxemia all the time (empyema, malignant pleural effusion). These patients often arenβt hypoxemic, more dyspneic, arguing the hypoxemia is a parenchymal driver.
23.02.2025 21:16 β π 2 π 0 π¬ 1 π 0Both of these points are relevant but diuresis is still king in both situations. Pleural intervention without diuresis will result in more pleural accumulation. A better tact is to diurese first, then consider pleural drainage later. Limits repeat interventions.
23.02.2025 21:14 β π 2 π 0 π¬ 2 π 0Hard to argue removing fluid outside of the lungs will improve the hypoxemia. Mechanics yes, diffusion defect no.

What if we just decided to help everybody
And not be worried about βfreeloadersβ
Like we just agreed that our North Star would be that everyone thrived
Man that would be awesome

10.02.2025 17:33 β π 2 π 0 π¬ 0 π 0Hereβs a reason why the incoming NIH Director, health economist Jay Bhattacharya, should know cutting indirect costs at NIH is self-defeating and stupid.
Every $1 invested in NIH returns $2.5.
So $4B βsavingsβ is actually $10B we lost.
Photograph of a warehouse in Kinshasa where unboxed tuberculosis medications are going undistributed due to the stop work order by the Trump Administration.
Just received this photo from a friend of a warehouse in Kinshasa. These are tuberculosis medications--ALREADY PAID FOR--that aren't being distributed due to the Trump Administration's stop work order.
TB treatment is being interrupted in SO many patients around the world. What does that mean? (1/2)
Ah, the latter was the context I needed. Thank you.
