it's all just assignment and correlations. sometimes those correlations are better than others, e.g. answering yes to "do you get a period" almost certainly means you have a uterus. but they're not all like that.

an uncomfortable truth here is that there's just not a 1:1 relationship between patient histories and, like, medical status. if someone asks you whether you have a prostate, the correct answer is "i don't know" unless you've had imaging done or palpated it yourself for some reason...

...is that an intake person looks at the forms, looks at the patient, and makes a judgement call about whether they need more information or just enters whatever they think is correct into epic. it's assigned sex the entire way down.

if i were a betting woman—which i'm not—i'd bet the above describes ten times more people than the number of transgender patients you're going to see in your average surgery.

the average person knows "i'm a he" or "i'm a she", and that's obviously not medical information. what really happens...

asking patients questions like "do you have a prostate/uterus" is also of limited utility, insofar as more patients than you think don't know those words or what kinds of people have those organs, let alone whether they do. there's plain language like "urinate (pee)" on UK medical forms for a reason

I think people think we're exaggerating when we say the widespread use of the term "biological sex" as a term itself isn't organic. This data isn't anomalous, it comports with everything we've seen about the specific use of the term.
