I’m so frustrated with Board and Care facilities pushing their own hospice or the hospice giving them kick-backs to convince the unknowing patient/family that this particular hospice is the only option. I also see this at larger ALF’s. #HAPC #hospice #geriatrics

Yes I do the same, but I have so many patients who are declining, are bed bound but still speaking (even if incoherently) but clearly have a prognosis < 6 months.

…and sit up but still speak in sentences even if incoherent or answer questions but demonstrate clear confabulation. Technically they don’t meet FAST 7A criteria but do for 7C and 7D. Assuming they are eating less, losing wt, have had a recent UTI and clearly declining what FAST score do you use?

I’m hoping this post gains some traction as I’m curious what others may be doing. Maybe the docs at the @geripal.bsky.social can give some insight. I struggle a little with the FAST score for advanced dementia patients. I’ve had patients in the past who lose the ability to walk… #hapc #hospice

It so sad that a patient who has advanced malignancy and has been in our system for many years, has no psychosocial or spiritual history. I’ll be the first to document this today. #hapc #palliativecare
