Near-infrared spectroscopy during mechanical Thrombectomy. I am trying to classify relationships between hemispheres (affected vs. unaffected). Did anybody have experience with it? #stroke #thrombectomy #spectroscopy #cerebral

In patients with salvageable brain tissue identified by perfusion imaging who did not initially receive thrombectomy, alteplase given 4.5 to 24 hours after acute ischemic #stroke onset may improve functional outcomes.

The treatment works, but kids can’t get it
Terry Pirovolakis, a former IT professional from Canada, moved mountains and raised a fortune to shepherd a gene therapy for his son’s rare disorder through development. Now he is the go-to contact for other families facing the same dilemma: science has discovered the therapies and delivery technologies to treat their children, but it’s not profitable for drug companies to make them. Pirovolakis eventually founded his own company, Elpida Therapeutics, to bring such treatments to approval β€” but it relies on a patchwork of fundraising, tenuous grants and an obscure US government voucher program. Meanwhile, children’s health is slipping away. β€œI think what he’s doing is just extraordinary,” said FrΓ©dΓ©ric Revah, CEO of the French gene-therapy nonprofit GΓ©nΓ©thon. β€œBut the whole field cannot depend on guys like Terry.”

Useful read on statistics and o values!πŸ§ͺ🧠

In my opinion (as a user, not an statistics expert), the p value is a tool to agree that something may be going on. Without context or further confirmation by independent experimental approaches, it means nothing.
It is not sacred evidence of an effect.

Agree. Instead of reinventing the wheel we can still use plain old CT+CTP to detect acute and relevant lesions in patients treated with ATT and presenting with emergent symptoms. Still waiting that someone prove otherwise. Cheers

Common gene variant APOE4 offers clues for new Alzheimer’s drug A study of the impact of a genetic variant that can increase Alzheimer’s risk, APOE4, could clear the path for creating an effective drug.

Stanford Medicine neurologist Michael Greicius, MD,
spearheaded a recent groundbreaking genetics study focusing on Alzheimer's disease. This study examines the impact of gene variants, particularly APOE4, on Alzheimer's risk and treatment responses.. stanmed.stanford.edu/alzheimers-v...

Neuronal and oligodendroglial, but not astroglial, tau translates to in vivo tau PET signals in individuals with primary tauopathies - Acta Neuropathologica Tau PET has attracted increasing interest as an imaging biomarker for 4-repeat (4R)-tauopathy progressive supranuclear palsy (PSP). However, the translation of in vitro 4R-tau binding to in vivo tau P...

Massive paper alert: We performed extensive profiling of PI-2620 for 4R tauopathies. PI-2620 tau PET signals in PSP stem from neuronal and oligodendroglial tau, not astrocytes. Signals correlate with fibrillary tau, autoradiography, and autopsy data in PSP

I agree about binary outcomes. However, it is more often the case to encounter one-way reasoningβ€”comparing two features that are, by their nature, interval variables and arbitrarily choosing one to binarize or categorize. I have done this myself as well. Perhaps it is time to reconsider.

Kudos to the authors on an excellent paper! It is a remarkable undertaking, though at times dense to read. As the authors note, PET-CT was considered the ground truth, albeit with several caveatsβ€”it provided a binary outcome without quantification. #Alzheimers #Biomarkers

New paper alert: A two-cutoff CSF AΞ²42/40 approach improves AΞ² detection in real world clinical settings. CSF AΞ²42/40β‰₯7.1 rules out amyloid, while ratios~5.5–7.1 benefit from AΞ² PET. Key for Lecanemab stratification. #Alzheimers #Biomarkers

An interesting clinical case seen yesterday: a man in his early 20-ies experienced sudden room tilt of 90-120 degrees alternating for 30 minutes long with associated autonomic symptoms. Presenting after 10 days still having occasional dizziness but without residual deficits. What is your diagnosis?

It is time to start a conversation about alternative imaging strategies for patients undergoing anti-amyloid therapy. Is MRI always feasible? Could CT multimodal imaging serve as a viable alternative? The tide is rising, and we must be prepared.
