Kidney biopsies are recommended if CNIs have to be continued > 2 yrs in children, to exclude toxicity. Side-effect profiles may drive CNI choice as gingival hyperplasia & hypertrichosis are more prevalent with cyclosporine & glucose intolerance occurs more frequently with TAC. #NephJC
Low risk of tac balanced out by real risk of infection and thrombosis in uncontrolled nephrotic syndrome. People forget that 80-90% of these kids died pre-steroid. #NephJC
