VC mostly funds primary care startups that seek to create vertical integration across services (eg. pharmacy + primary care). The missing piece: vertical integration within primary care - eg. team-based care where a patient can have just-in-time care from BH, dietician, PharmD etc.

Carbon Health Bankruptcy Marks End of 'Tech Enabled Primary Care' Startups | Noah Weiner posted on the topic | LinkedIn Carbon Health’s bankruptcy announcement this week marks the end of β€œtech enabled primary care” startups. This is how they ended up: 1) Three went bankrupt or closed down (Babylon + Forward + Carbon) 2) Two ultimately exited to Amazon (Iora Health + One Medical) 3) Two went to retail pharmacy (VillageMD + Oak Street Health) I know Oak Street was a financial winner at the time, there is always luck involved, but it’s a little bit of a wash today. CVS acquired them for $10.6B and has since taken a $5.8B write-down and is closing locations. Walgreens poured $6.2B into VillageMD and is now shutting 160+ clinics. From my rough research ~$3B in VC funding and ~$6.5B in corporate capital went into funding these businesses for a total of ~$9.5B. And what do we have to show for it? They ended up at a shipping company (Amazon) and retail pharmacies (CVS and Walgreens). These are companies built to move packages and fill prescriptions, not transform how care is delivered. What’s starting to become clear is how much time was spent by the executive teams solving problems of financial engineering (how can you not with $100M of debt on your books) rather than clinical engineering. Focus on solving the real problems of the business. What’s also clear is we are still very early in the innings of AI, so none of these companies were able to make the shift from β€œtech enabled care” to β€œAI enabled care.” Time to go back to the drawing board on how to actually build a lasting healthcare company.
