The Clinical Research Flywheel: From Volume to Discovery
How surgeon expertise and patient volume create a self-reinforcing ecosystem for generating increasingly sophisticated research questions
Why are 30% of pediatric tracheostomy patients Black when they're only 15% of kids?"
That question only emerged because we compared local data to national data.
Here's the framework for how one observation generates increasingly sophisticated research: open.substack.com/pub/pediairw...

The thing that jumps out to me is still the training of the agents. Whose values get encoded. There remains a cultural hegemony even the probability space.

I asked Claude to map your idea, but added my own twist as a surgeon. That is, AI is "work in the head" and that is its ongoing limitation.

In Trump’s Ideal Picture of America, Diversity Is Taboo
This piece exemplifies how journalism can serve democracy by recognizing when standard practices (treating each policy as an isolated event) fail to capture systematic threats to democratic norms.

It starts with a perfect circle of universal rights. Then, "We must act outside of the law to save it." The erosion is not a single, dramatic event, but a creeping, iterative process until universal protection under the law becomes a fractured reality where the boundary of justice is arbitrary.

Kennedy Cancels Nearly $500 Million in mRNA Vaccine Contracts
mRNA can produce vaccines much faster than older methods, which are critical in pandemics. Cancelling them only increases the risk when the next pandemic comes. Not to mention vaccines for treatment of cancers and other immune-sensitive disorders.

Columbia and Brown to Disclose Admissions and Race Data in Trump Deal
What's fascinating is how merit gets defined & redefined to maintain a particular hierarchy. Also, we've taken someone's humanity & drilled it down to a single test. It's like buildinf a NFL football team based solely on what the players do at the combine. www.nytimes.com/2025/08/05/n...

"In a time of universal deceit, telling the truth is a revolutionary act." Great quote, though apparently Orwell never actually said it. Funny how the best quotes are often misattributed.

Fascinating to see the inner mechanics of AI training in real time. If you use ChatGPT 4.0 for healthcare queries, it uses American English spelling. Switch to 3.0, and then it defaults to British spelling…
