AI scribes may not save a lot of time on clinical notes, but they make clinicians happy As hospitals study the impact of AI scribes, they are finding that clinicians love the tools, even if they donโ€™t save that much time.

Opinion | A sickening moral slum of an administration Regarding Venezuela, Ukraine and much more, Trump and his acolytes are worse than simply incompetent.

Conservative columnist George Will gets it.

In a piece titled "A sickening moral slum of an administration," Will writes:

"Defense Secretary Pete Hegseth seems to be a war criminal. Without a war. An interesting achievement."

Senator Kelly is correct.

Plus a Public Service Announcement:

KEY section of Department of Defense's Law of War Manual is 18.3.2.1.

The VERY rule on the "requirement" to refuse illegal orders gives as its paradigmatic example of what is "clearly illegal:
"orders to fire upon the shipwrecked."

FDA review staff was excluded from voting on whether to approve first priority voucher drug Top officials like Vinay Prasad and George Tidmarsh voted on the drug, a major break from the FDA's typical practice.

โ€œYou are taking the decision away from the reviewers, and youโ€™re putting it in the hands of the political leaders of FDA,โ€ Fernandez Lynch said.

It doesn't take an expert to understand how bad this is. More great reporting from @lizzylawrence.bsky.social

Service members do have a duty to refuse illegal orders

Trump has murdered at least 83 people with extrajudicial military strikes on alleged drug-trafficking boats.

He needs to face consequences for this.

His secretary of DHHS, RFK Jr., will kill several orders of magnitude more with this disinformation.

Vaccines do not cause Autism*
Pursuant to the Data Quality Act (DQA), which requires federal agencies to ensure the quality, objectivity, utility, and integrity of information they disseminate to the public, this webpage has been updated because the statement "Vaccines do not cause autism" is not an evidence-based claim. Scientific studies have not ruled out the possibility that infant vaccines contribute to the development of autism. However, this statement has historically been disseminated by the CDC and other federal health agencies within HHS to prevent vaccine hesitancy.

HHS has launched a comprehensive assessment of the causes of autism, including investigations on plausible biologic mechanisms and potential causal links. This webpage will be updated with gold-standard science that results from the HHS comprehensive assessment of the causes of autism as required by the DQA.

The following, as required by the DQA, details the state of the evidence and studies, and the lack thereof, regarding vaccines and autism spectrum disorder (autism) and outlines HHS future research directions to provide answers.

However, SONIA trial7 proves that there is a far more efficient way to answer these questions, by running a superiority rather than a non-inferiority trial. The burden of proof in medicine lies on more treatment. Primum non nocere dictates that the burden of proof should lie on doing something or doing more, rather than on doing nothing or doing less. In other words, if the standard of care is no treatment, use of a new drug should prove to be better than doing nothing; if the standard of care is to use one drug, combination treatment should prove to be better than the single drug.

Interesting article out this week from Dr. Bishal Gyawali, arguing that we should use more superiority designs for treatment de-escalation studies in cancer.

I have a number of colleagues who think that LLMs will be very useful as individualized tutors.

One of their arguments is that unlike previous generations of AI tutors, LLMs have the full context window of the conversation and thatโ€™s better understand what a student knows and doesnโ€™t know.
