Agree that’s a fair counter argument. Devil in the detail as always. We’ll have to wait see what actually gets proposed.

All I’m doing is pointing out that a small fee may not be so outrageous in most cases. System would speed up. Outrage seems to greet every attempt by labour to balance the books atm. Sometimes justified, but not here imho. (6 months to find it once workers rights bill passes 🤞)

Of course I understand. Means testing has previously been used, reasonable to assume it might be again. Not every employment tribunal case is David vs. Goliath you know. Waiting years to have your case heard (cos there no money) is bloody awful as well. Always a balance to be struck.

It all depends on the proposed fee. Fees were pretty high when coalition brought them in which WAS a big problem (see my other post for deets). No idea what $$ labour are proposing, it’s all speculation until then. Backlog of 45k cases due to lack of funds is not great for justice either.

(Although I don’t necessarily think it is a stupid idea. Depends on the level of the fee!)

This I agree with. Stop deciding on policy based on how it lands when you waft the idea in front of the press!

Ps there are plentiful judges! Just not enough funding for cases.

Slightly muddled journalism. Fees from 2013-2017 were between £400 and £1.2k to have case heard, plus other fees in some cases. Exemptions for those on low incomes, possibility of reimbursement if case successful. Still not surprising that cases fell through floor.

The saving per year is ~1000 tonnes of CO2e.
This is around 10% of the carbon footprint of the Trust.

People can consider for themselves whether that is too much to spend to enable this treatment, which is highly valued by very many midwives and women. 2/2

A real world example: the capital cost to install nitrous cracking in my local maternity unit (~6000 deliveries pa, quite a big unit by UK standards) is 300k. Ongoing maintenance costs roughly equivalent to existing scavenging system (which pumps the entonox out of the hospital chimneys) 1/2

We do not consent patients using entonox for bone marrow biopsies, endoscopies, burns dressings etc. etc. for the risk of global warming.

Maternity care should not be the exception.

Epidural or remi PCAS for all is neither desirable nor realistic.

We do not routinely highlight the ruinous environmental cost of healthcare ‘choices’ to other patient groups. It does not form part of the consent for robotic surgery, for example, or admission to intensive care.

Why shouldn’t women be able to access entonox in labour, if all it takes is ‘substantial capital, infrastructure, energy and ongoing maintenance’ to mitigate environmental harm? Show me modern European healthcare that does not require these things?

ICU sedation, or ‘anaesthesia-lite’, is definitely harmful. It’s acceptable only insofar as it’s unavoidable and/or the least bad option. Not comparable with what we can and should achieve in theatres.

Weirdly seems to only affect tickets from Ben Rhydding on the Ilkey-Bradford line. Train tickets should not be this complicated! On the bright side, at least I know now! 🚂

Bah! I often commute from Ben Rhydding to Frizinghall. Did not think advance tickets were available on the line (never an option). Turns out buying the ticket either onto Bradford Forster Square, or alternatively, from Ilkley station, gives advance fares saving > 50%. Bloody Northern Rail!

All 12 patients obeyed commands when they were supposed to be anaesthetised. Conflating following motor commands with consciousness is pretty standard. But maybe….this assumption is misleading? Sleepers will often obey simple commands, particularly in REM cycles. Are they conscious?

Consider humans without the capacity for explicit (important distinction) memory eg. young children, dementia patients. Implicit memory can still result in profound psychological distress without recall. Therefore, for me, it becomes very easy to say that it is not acceptable to be aware under GA.
