Ohio is almost entirely part of Appalachia. I would not call most of it Midwest, especially as that group expands to the West.

I'll defer to you experts on the rest. There is a distinct culture shift from the agrarian Midwest like Iowa, Kansas, and Nebraska compared to Minnesota and Wisconsin.
