A form with two fields; email address and phone number. The user has written “no phone” in the phone field; this has triggered an error because it’s not a valid phone number. The user cannot proceed.

A really good point by @kitation.co.uk at #SotB26 that form validation can make forms inaccessible if it’s not thought through - by forcing a phone number field to be a valid phone number, you’re not giving people the option to say “I don’t have a phone / I can’t use a phone / please don’t call me”
