The only hope for our field is triadic care, where AI is deployed in such a way that maintains -- or even enhances -- human relationships. Otherwise, "AI doctor" startups will continue to chip away and further fragment our already struggling primary care.

The problems in scientific publishing run deep. They won’t be fixed by courts or regulators.

If the system is going to change, it will be because scientists choose fairer journals, nonprofit models, and better alternatives.

A federal judge dismissed the case, not because the system is fair, but because current antitrust law isn’t built to regulate academic norms, trade-association ethics rules, or structural power in publishing.

Scientists sued the six biggest for-profit academic publishers, alleging they colluded to enforce “publish-or-perish,” single-submission rules, unpaid peer review, and limits on sharing research. The results just came in…

The top six commercial publishers are currently being sued for anti-trust violations (Uddin v. Elsevier, B.V., 1:24-cv-06409). They publish the majority of articles on PubMed. This year, we will find out the verdict.

If they lose, this would have DRAMATIC effects on #research

A randomized controlled trial of curated X exposure for cardiac point of care ultrasound education - PubMed Natural X exposure to educational POCUS accounts did not result in statistically improved image interpretation scores, but this outcome was likely affected by limited sample size, different participant experiences, and other variables. Our trial started in 2021 and the platform entirely changed mid-trial. Our enrollment dramatically slowed after the changes. Still, I firmly believe #MedEd needs more practical and comparative effectiveness studies, despite added challenges. pubmed.ncbi.nlm.nih.gov/41350857/

The results: we unfortunately only ended up with 29 participants, but the results were completely flat, not even a trend of better #POCUS image interpretation among participants following POCUS accounts. The big caveat to this result is obvious…

First, it was an unblinded randomized trial at 3 IM residencies and 2 medical schools, enrolling IM residents and 3/4yr med students. They were told to use it 2hrs/week for 3mos, completing logs, and either following 10 #POCUS accounts or 10 non-POCUS journal accounts
