If everyone agrees, no one cares and nothing matters, there is no conflict, but this rarely reflects reality. Nor is this desirable!

Conflict is normal, and healthy.

Mostly convention! And because we can’t monitor ICP and thus CCP during the course of surgery.

I suppose it makes sense in terms of maximal vessel wall stress in the context of vascular pathology too.

There’s not much high quality evidence.

Ultimately <160sys but if starting at 220 as in your example I’d try to get there gently, not in one big immediate drop.

Signal reduction on fMRI, I wish there was a clinical measure in here too. Interesting that indicates memory affected.

Anybody out there used IT morphine for analgesia post spinal surgery?

Epidural LA depot flooded in by surgeons can be useful and is used with varying deliberacy in some UK centres. I know of some teams who close leaving an epidural catheter in, then bolus LA down catheter and immediately remove.
