3/ people who had bariatric surgery years ago, mainly revert to BmI 35+ social factors, access to healthy nutritious food & more. Commercial determinants of health .

2/ but things have to change- bang on it’s about relationship with food and ingrained and habits. Evidence does show being physically active helps maintain a healthy weight, it has to be a part of long term management

Well done on your fantastic success, just reading your Observer article. We are bathed in GLP1 in practice at present & have people who had bariatric surgery 20-30 years ago. Also from friends who have been successful coming off it can be done without rebound weight gain. 1/

The flaws are not with clinicians and people but with systems & divisions. Especially between community and hospital processes. Negative impact on decision making & patient care every clinic, everyday. Great article @kamranabbasi.bsky.social @bmj.com

Reminds me of an old story from pre NHS era of GP calling through a patient with a broken leg to the hospital, knowing there were no broken limbs, he had a pneumonia- but fundamentally needed a hospital bed and would be declined if the admitting GP had been honest.

3/ if we’d had access to a scan in 2 weeks, there would have been no need for a scare. Current system doesn’t work for patients & frustrates clinical decision making.

2/ she came back with her husband this week for the result, she has always come alone before. It showed bronchiectasis, not a cancer which fitted with the clinical picture. She told me how scared she was when I’d told her we were getting the urgent scan.

1/Reflecting on a lady I’ve been seeing with a cough, added sounds on chest, I wasn’t worried about cancer from our conversations since Xmas, but to get a CT, <1 week on 2ww pathway, > 18 weeks on routine path- this isn’t good enough.
