usually, a prodromal diagnosis is given based on positive biomarkers, so a form of diagnostic counselling happens before the pt agrees to have the tests. If there is no dementia syndrome I would not say they have dementia, but the prognosis will be discussed openly.

Strictly speaking, ‘dementia syndrome’ should be diagnosed when there is evidence of ADL decline - though which ADLs is usually less defined and individual. But you can diagnose a dementia disorder-like Alzheimer’s disease- in the pre-dementia stage (prodromal and preclinical)
