The enzyme becomes overactivated, producing a lot of citrullinated proteins, which are detected by specific antibodies. When bound to the joint proteins, such antibodies activate immune cells. This leads to the tissue injury caused by active immune cells and specific proteins called cytokines.

Rheumatoid arthritis (RA) usually starts with local inflammation, leading to (often small) damage to joints. Damaged joints release negatively charged polysaccharides called glycosaminoglycans, which activate the enzyme PAD4 (the main enzyme responsible for biochemical changes in RA).

Interesting report shows that peptidylarginine deiminase 4 (PAD4), an enzyme responsible for citrullination of arginine in rheumatoid arthritis, is activated by glycosaminoglycans released from cartilage, which may exacerbate the symptoms.
