I mean, they moved all of the military assets into Iran's backyard and sat them there for weeks in hopes that they'd be attacked by Iran, or a proxy and given a pretense for a response.

The patient never requested anything so they aren't asking questions when no DME actually shows up.
DME is less sensitive (it's not like prescribing a narcotic) so the level of attention that request might get before being approved is relatively low, and billions of dollars of these scams get OK'd.

These fake companies submit pre-filled scripts to physicians requesting that they sign off on them to provide a patient with some sort of DME (braces, crutches, etc.)
Their hope is the physician doesn't fully review the record and just signs off (which is negligent and considered provider fraud).

It's literally just a very heavy handed fraud prevention effort.
CMS has been railing on this for years and there were unprecedented campaigns in 2025 trying to commit this fraud on a pretty significant scale.
The scams are sophisticated but provider due-diligence ought to be enough to catch them.

There has been guidance from HHS on this for years because of DME fraud where these fake companies try to get MD's to sign off on unnecessary braces, crutches, orthotics, etc. and falsely billing Medicare and Medicaid for it.
It's a real problem but I doubt this will fix it frankly.
