I’ve spent years, decades, obsessing over the Bush Administration’s Iraq WMD lies, writing about them, debunking them.

I can think of no lie about Iraq WMDs told by Bush, Cheney, Rumsfeld et al that comes even close to the enormity and absurdity of this lie from Donald Trump.

There was definitely a time, long before Elon's involvement, when Twitter had properly identified journalists with checkmarks and they absolutely would break news on Twitter itself.

Was always a mixed bag, as media frequently is, but it was legitimately valuable.

Newsroom certainly has a lot of room for criticism but it's still loveable.

That was transparently obvious when they started this hemming and hawing over why the assets were even there.

Yes - but mostly from Medicare/Medicaid (which act as an insurance company) for the people impacted.

The patient never requested anything so they aren't asking questions when no DME actually shows up.

DME is less sensitive (it's not like prescribing a narcotic) so the level of attention that request might get before being approved is relatively low, and billions of dollars of these scams get OK'd.

These fake companies submit pre-filled scripts to physicians requesting that they sign off on them to provide a patient with some sort of DME (braces, crutches, etc.)

Their hope is the physician doesn't fully review the record and just signs off (which is negligent and considered provider fraud).

It's literally just a very heavy handed fraud prevention effort.

CMS has been railing on this for years and there were unprecedented campaigns in 2025 trying to commit this fraud on a pretty significant scale.

The scams are sophisticated but provider due-diligence ought to be enough to catch them.

Any end to this that isn't putting them in the camps they built is short of success.

There has been guidance from HHS on this for years because of DME fraud where these fake companies try to get MD's to sign off on unnecessary braces, crutches, orthotics, etc. and falsely billing Medicare and Medicaid for it.

It's a real problem but I doubt this will fix it frankly.

Terrifyingly, those making these decisions are some of the least trustworthy and worst role-model humans around.

It's insane to me that any business is willing to take on the potential legal liability while these wildly immature and dangerous tools continue with no meaningful regulatory guidance or precedent for legal liability or accountability.

Which is doubly true for any developer of these tools.

They apparently haven't considered accountability for anything either.

A programming team screws up catastrophically and you can hold the humans involved responsible.

Who's taking the blame when agentic AI with relatively little human oversight or input starts causing real harm or business loss?
