MRS did take the crown in this head-to-head comparison. For now, however, the recommended MRI protocol does not include MRS. I hope that the scientific community can continue to gather data to motivate its future use in concussion.

Hi Aurora - diagnosis now does include more biomarkers, compared to years past. Imaging has a central role, and other objective measures can also be taken into account. More info here: pubmed.ncbi.nlm.nih.gov/40409315/
How diagnosis is actually done, however, is still clinic-specific.

Multi-modal proton and sodium MRI for outcome prediction in mild traumatic brain injury
In our recent MRI study of concussion we found that MR spectroscopy (MRS) fullfilled the most criteria for a clinical biomarker. The comparison was between #MRS, the common technique of diffussion tensor imaging, and the more novel sodium MRI and MR fingerprinting.
