JC: Do physician led prehospital teams improve outcomes? What is the impact of physician-led interprofessional pre-hospital teams on mortality outcomes for critically ill and injured patients.

Not that I can add much to this excellent description by @emmanchester.bsky.social, but there's a touch of a confounder.
The biggest model we have for "non physician HEMS" is also the country that has some of the lowest standards for EMS education.

This point on preparing for #PHEM application is equally valid for #paramedics aspiring to specialist practice

Spend majority of time building your CV doing things you enjoy to meet person spec

If you donโ€™t get in then you wonโ€™t feel like it was a waste of your time

Donโ€™t forget these physician lead model are selective on what they dispatch themselves to. Unlikely to attend frail, co-morbid or non shockable cardiac arrest etc. canโ€™t really generalise when itโ€™s not an RCTโ€ฆ. Heavily selection bias

This article rings true everywhere. It might be more pressing in the UK, as other countries willingly accept their senior staff with better pay and/or work hours, but it's still a factor in rural areas of the US and Australia.
And certainly worth a read.

Notable difference from moving from a NHS ambulance service (struggles to retain experience) to New Zealand (keeps very experienced staff) in terms of productivity, decision making and overall patient care!

Very thoughtful and nuanced post from @garethgrier1.bsky.social on the huge complexities of potentially delivering extracorporeal CPR (ECPR) across the U.K. There is much enthusiasm and pockets of excellence - but a whole system-wide change is needed. Like REBOA and RT, a HALO procedure (1/2)

A total pain prevalence of 51% was found in New South Wales, w 5% of patients stating that their pain was not managed.

People with indigenous backgrounds & who spoke a language other than English were less likely to report adequate pain management.

It takes a system to save a life in cardiac arrest. Iโ€™m proud to have played a small part in this paper in The MJA which shows how survival has incrementally improved in Victoria over 25 years and highlights the value of a registry. If you donโ€™t measure, you canโ€™t improve ๐Ÿค“ ๐Ÿš‘
