When a medical student I remember a physiology practical in which we had to put a tourniquet on one arm. After about 90 mins I was able to stick pins in my hand without any feeling at all - that was until the tourniquet was released π€£π€£

Questioning drives learning and understanding.
If we were able to move to a state in which everyone who works in healthcare are seen as equally valuable we would be able to move away from this toxic culture.
We also often do a poor job of calling it out when it happens.

I am currently preparing some teaching for later this week. I like to start teaching sessions by reminding residents that the teaching session is a completely safe area and no one should feel anxious about answering or asking a question or questioning me if they think I have made a mistake. 2/3

It is tragic that we are still in this situation. Not only does public humiliation have negative mental health impacts it also affects cognitive thinking and medical decision making subsequently often when in an (unrecognized) emotionally similar situation in the future.

The issue of βnormal rangesβ is a big problem. My Hosp gave 10-50 as normal ferritin. My Nathan & Oski (5th Ed) says females Ferritin range (15-16 yrs) mean =18.2 and Nelsonβs suggests ref range for females of 10-70 which leads people to think anything over 10 is normal which it clearly is not.

Bald wrinkly retired Pediatrician here - remember LPβs virtually every shift, terrifying epiglottisis meningococcal sepsis with limbs lost, kids with neurological disability and hearing aids in follow up clinics. Kids with SSPE. We need to advertise these appalling outcomes as widely as possible.

All residents regardless of specialty should rotate 1-2 weeks through laboratories to see how they function instead of phoning them up demanding results

I used to take residents to the labs all the time to show them how they function and to educate them on the wealth of knowledge and high level of training laboratory staff have ( + keep my hand in looking at π©Έ films!) Good communication/collaboration with laboratories is crucial for Pt care. 1/

As a Peds onc resident many years ago I remember doing bone marrows on twin infants with pertussis pneumonia in ICU as both had WCC >50 to exclude BM diagnosis. No underlying marrow disease or immunodeficiency. They both very sadly passed away. An awful infection in infants & young children
