January resolutions, because one step at a time.

- No buying food/ coffee at work.
- I will not read the news on my phone.
- I will put my phone away in the evenings, not in my pocket.

Yes I’m aware of the irony of coming back to SoMe shortly before resolving to use my phone less…

Can we all agree that the New Year should start on 2nd January?

The 1st should be more of an inset day.

Use it to get ready, to do what you need to.

I’ll be my perfect self tomorrow…

I think you’re spot on Shaun.

We know the problems.

Workforce planning
Funding
Maladaptive responses when the first two don’t exist.

We don’t need an enquiry, we need meaningful investment and strategy for how that money is best used. (IMHO).

We have a working switch 1 and I’m trying to teach SmallestOne (7) how to play racing games (sonic racing).

I’m not sure this is my finest parenting moment.

I’m trying to explain that the joystick turns the car, not him moving his entire body…

For new year next year, I would like our Emergency Department EPR to give me an ED Wrapped for the year.

“Congratulations, you saw 1221 patients this year!”

“203 of them had no abnormality detected.”

“You made 1834029274902 senior reviews.”

“Your favourite procedure was ketamine sedation!”

“Errrr… Mr Fleming, we noticed your list ran over quite a bit today, do you have any idea why that might have been?”

I’m going to enjoy the replies to this 😂

It’s definitely not continued to ramp up in the way we feared, not sure I’d use the Q word though, definitely still busy.

Bridget the cat infiltrates the 'Abbey Road' cover shoot and becomes THE definitive fifth Beatle.

THREAD.

My parents' cat Bridget vanished. As the weeks dragged on they became ever more worried, so to distract himself my dad began to paint Bridget's adventures, imagining her travelling through time and popping up at some of art & music's most important moments.

I've collected his work here...

Life goals:

Smallest Two (2) is sat on the hallways floor reading Smallest One’s (7) comic books.

She’s been there for 30 minutes just chatting to herself and flicking through pages.

I’m absolutely desperate for something that isn’t chocolate or cheese…

I’ve drunk so much tea I think I need dialysis.

I’ll save this quote for you for when we are both in our 80s and being mistaken by the nurses for escaped patients while doing our ward rounds.

It’s going to be risky that I have to get through OPED to ChED as some well meaning nurse tries to guide me back to a room 😂

Reclaiming FND for Rehabilitation Medicine – new open access editorial

Neurorehab colleague Phil Milburn-McNulty and I set out why Rehab Medicine is so well-suited to treating FND, why they historically haven’t and what can be done to change things.

I wonder how many of our systems are built to take us away from having eyes on the patient, and are confusing to navigate to the point where we get it wrong.

EPRs, referral systems, education buildings off site. All in some ways remove us from where the patients are.

I might be a little late to this bandwagon. If you have time, please show your support for the Veterinary education team at Cambridge University.

I’ve had the joy of working with some of their amazing faculty & I cannot fathom why anyone wants to stop their exceptional work.

Did you see @uhsft.bsky.social's @apsmunro.bsky.social on BBC South with @edwardjsault.bsky.social? 📺

While many people recover quickly, flu can be dangerous for young children.

A new trial is exploring whether the flu vaccine can be extended to younger toddlers.

100%

And of course patients can move, but our patients in particular have clear barriers to doing that.

Shared care agreements create a postcode lottery by GP practice, and in the long term we need a better system.

It seems more and more GPs are refusing to take on shared care with specialist teams for #ADHD.
This makes sense if you think of a GP practice as a small business needing to maintain profit margins.
But most small businesses allow their customers to go elsewhere for a service...

ICB colleagues can be really helpful in these situations.

I think this also speaks to the increasing complexity of GP workload. They are consistently overwhelmed and it is easier just to say “no,” as all shared care agreements are discretionary.

The solution has to come from both sides in a positive and constructive way, with the funding to support.

Functional neurological disorder Information about functional neurological disorder (FND), including the symptoms, causes, and treatments of this condition. For patients in Scotland.

2. Patients are often (still) being given the FND diagnosis when “no disease is found.”

This is not how it should be done.

FND needs to be diagnosed based on positive signs / clinical features that establish that FND is present.
