(3/4) Our goal is to work with shelter and long-term care home (LTCH) partners to co-develop & pilot a #LTCH model to transition these individuals from shelters to LTCH while receiving tailored #geriatric care, and to evaluate impact on outcomes that are important to older adults.

(2/4) More than 25% of people experiencing homelessness in Canada are older adults; in Toronto alone, >3000 adults older than 50 are #homeless. They're at higher risk of adverse outcomes such as death compared with people who have stable housing.

For #EmergencyNursingWeek, we are highlighting our work on the fascia iliaca compartment nerve block (FICB)!

The KTP assisted with the development of an education tool for patients on FICBs, and a tool for Emergency Department nurses for their role in the FICB process.
