Twist bones are great for avoiding that "toothpaste tube" effect! You have two deformation bones in the same place as the arm that are constrained to the hand (here at .3 & .6) while the "real" arm bone isn't weighted at all!

Two important features for advanced rigs are "twist bones" and "bend bones". These cool little features prevent the mesh from collapsing in on itself when it flexes and allows for more natural looking deformation!
