Statins enter every cell in the body including the brain as they can pass thru the BBB. Statin's CV benefit comes from reducing apoB, by inhibiting HEPATIC cholesterol synthesis which in turn upregulates LDL receptor & the liver, has far more LDLRs than other peripheral tissues.

There are 2 forms of cholesterol - An alcohol ending in -OL (the active "free" form) & an ester ending in -YL (the storage/transportation form). The esterifying enzymes attach an acYL group (fatty acid) to free cholesterol are ACAT 1 & 2 in specific cells & LCAT (alpha and beta) within lipoproteins
CIMT can be even more important than CAC as vascular pathology appears before calcification. A significant proportion of low-risk individuals may have undetected plaque, justifying reconsideration of early screening strategies. Open access at pubmed.ncbi.nlm.nih.gov/41449009/

This is a 9-minute podcast in the famed Lipid Curious series by Visha Priya Pulipati that sort of positions who I am and how I got there. I stumbled into and tried to master the importance of lipids before most clinicians. Enjoy. www.buzzsprout.com/2508586
More of my teaching slides to help all better understand lipid and lipoprotein complexities. Wish you could view the animated versions. Note: lab lipoprotein cholesterol assays measure both free and esterified cholesterol. See if you can figure out why one of obicetrapib's trials was called TULIP.

Part of the HDL story not usually taught: After receiving TG from apoB particles via CETP, HDLs undergo lipolysis via the actions of hepatic & endothelial lipase. Ultimately the products of lipolysis are very small HDLs & apoA-I which are handled by the Megalin/Cubam complex in the renal tubules

These HDL slides illustrate some of the many complex pathways used by HDL's as it contributes to cholesterol homeostasis. Of course, the HDL-C metric tells you nothing about whether these pathways are occurring or not. If you are using any ratio that includes HDL-C, please cease immediately.
To help all better understand lipoprotein concentrations. Moles are a way of counting #s & 1 mole is the # of particles or 10 to the 23rd power & a nanomole is a billionth of a mole or 10 to the 9th power. HDLs BY FAR are the most numerous particles in plasma & VLDLs & IDLs are by far the fewest.

Here is another of my graphics: this time on atherogenesis. The sine qua non means there cannot be atherosclerosis unless there are cholesterol molecules in the arterial wall. Over a long time, they are delivered by apoB-containing vehicles called lipoproteins, the vast majority of which are LDLs.
Here is a learning experience on a crucial topic to put at the top of your list. I so enjoyed listening to two of my favorite colleagues discussing the brain during the menopause transition. @_CynthiaThurlow and Dr Kellyann Niotis www.cynthiathurlow.com/podcast/epis...
Had 2 hours with Dr Frederick Feit on SiriusXM radio this morning. I spun a lot of lipids/lipoproteins & took several calls. If you are a subscriber to that channel, the chat is archived there for relistening. www.siriusxm.com/player/episode-audio/entity/98cd4f0e-aad6-e0ae-566c-77c64cd526c7
Tomorrow from 6-8 AM, on @SIRIUSXM I will be a guest of famed cardiologist Dr Frederick Feit on Doctors Radio channel. I am the official historian and "Honorary Battalion Chief at the 211-year old Paterson NJ Fire Department. See www.patersonfirehistory.com/pfh-contribu...
Set your alarm clocks - I will be interviewed by renowned cardiologist Dr Frederick Feit on SiriusXM Doctor's Radio (Channel 110) live between 6:00 and 8:00 AM EST next Wednesday, January 21 (repeated periodically). Discussing what else - lipids, Menopausal Health & answering caller's questions.
