Statins enter every cell in the body including the brain as they can pass thru the BBB. Statin's CV benefit comes from reducing apoB, by inhibiting HEPATIC cholesterol synthesis which in turn upregulates LDL receptor & the liver, has far more LDLRs than other peripheral tissues.

There are 2 forms of cholesterol - An alcohol ending in -OL (the active "free" form) & an ester ending in -YL (the storage/transportation form). The esterifying enzymes attach an acYL group (fatty acid) to free cholesterol are ACAT 1 & 2 in specific cells & LCAT (alpha and beta) within lipoproteins

Part of the HDL story not usually taught: After receiving TG from apoB particles via CETP, HDLs undergo lipolysis via the actions of hepatic & endothelial lipase. Ultimately the products of lipolysis are very small HDLs & apoA-I which are handled by the Megalin/Cubam complex in the renal tubules

To help all better understand lipoprotein concentrations. Moles are a way of counting #s & 1 mole is the # of particles or 10 to the 23rd power & a nanomole is a billionth of a mole or 10 to the 9th power. HDLs BY FAR are the most numerous particles in plasma & VLDLs & IDLs are by far the fewest.

Here is another of my graphics: this time on atherogenesis. The sine qua non means there cannot be atherosclerosis unless there are cholesterol molecules in the arterial wall. Over a long time, they are delivered by apoB-containing vehicles called lipoproteins, the vast majority of which are LDLs.
