The reputation of the US Department of Health and Human Services has been in a dramatic free-fall for the past several months. Its gender dysphoria report will surely make that worse. Rather than undertaking a serious analysis of the medical literature, HHS assembled a group of report authors that included a political scientist from a right-wing think tank. The report contains an egregious lack of understanding of psychiatric medicine. For instance, the report claims that that gender dysphoria is unique because the “diagnosis of gender dysphoria is based entirely on subjective self-reports and behavioral observations, without any objective physical, imaging, or laboratory markers." This is true for nearly all psychiatric conditions, including major depressive disorder, generalized anxiety disorder, autism spectrum disorder, and obsessive-compulsive disorder, to name just a few. Surely the psychiatrist authors of this report should have known better. The most charitable assumption is that their work was rushed, as they note they were forced to be on a quick deadline. HHS wanted the report to be out ASAP, presumably to support the political goals outlined by Trump’s executive order related to gender dysphoria.
