Are motor-sparing approaches universally advantageous or are their benefits more limited and contingent on careful patient selection?

Here is a tiered, patient-centred decision model about prioritising function or analgesia.

Image of a medical professional positioning an oxygen mask on a patient lying down. Text reads: If sustained exhaled C02 is not detected, then oesophageal intubation must be excluded.

🚨We have retired "no trace, wrong place" to align with PUMA guidelines.

Instead, with @assocanaes.bsky.social and @dasairway.bsky.social, our message is that if sustained exhaled CO2 is not detected, then oesophageal intubation must be ruled out.

Resident opinion: In UK unworkable as a separate rota. Core skill, at least for anyone on an on-call rota, but hard to get adequate consistent exposure during training to build mastery. If there is a resident in the building they should be at least offered the chance to watch.

Yep this, little finger/back of left hand stabilizes between back, with thumb and index finger pinching wing and working as a brake to control speed of advance.

The Bluesky Anaesthesia feed has been updated to include posts matching common anaesthetic keywords from a list of accounts from anaesthetists/anesthesiologists.
Still using the Hacker News algorithm to balance sorting by popularity/recency.

One of the differences between Twitter and Bluesky is that 'likes' don't count for anything other than encouraging the person who wrote the post. It doesn't boost views or 'upvote' the post on followers TLs. If you want to disseminate, you have to retweet or quote tweet.

Surely this just reflects 'absence of evidence' rather than 'evidence of absence', given the extremely low incidence of aspiration (1 in 4K in elective surgery).

The authors acknowledge that risk could have increased by up to 1 in 2K cases. A 200% increase in risk of commonest cause of anaes death.

IJ: Great Circle has been fab, almost reminiscent of traditional point and clicks in pacing and story telling, but modernised. Must give AK a go some time.
