Four heads

Ever since I was a child, I've been paranoid. I thought my suffed animals conspired against me at night. Everyone in my life, including my parents, was lying to me in order to take something important and necesarry away from me... My body... My mind... my essence...

In order to simply survive, I had to find a way to know exactly what people were thinking. I studied everything I could about body language, tone, inflection, neural processes and the like. I got so good, I could essentially read minds. He reads a woman's mind. She thinks "I'm nailing this anecdote." Things were good for a while, but eventually that comfort eroded because I became convinced that even the most superfical thoughts of others belied a secret hatred of me. A passing man's thoughts transform from "I'm going to get a pizza" to I am coming to kill you."

Being able to read their thoughts was no longer enough. I had to fully control them. Beyond all logic and reaso, from the deepest recesses of my willpower, I reached out across the psychic plane and attempted to manipulta the mind of a passer-by. A passing man thinks "I need to pick up beer." It backfired. A little arm emerges from the inside of the man's skull and cracks it open. An evil little anthropomorphic beer can with sharp teeth erupts in bloody viscera from the man's head and starts chasing the paranoid man.

I was not being chased by a creature borne from the combination of an innocuous thought and my paranoid projection. What's worse is I couldn't turn it off. He runs away from the beer can through a crowd of people. Little arms start emerging from their heads. I somehow managed to evade the army of paranoia and I live out here now... alone in the desert... safe from a passing thought.
