Kusuriya no Hitorigoto is 100% realistic. It also provides good information about pharmacology (the protagonist is an apothecary).
Ron's cases are 99% realistic;the only fantasy element is Ron's ability/illness, which serves as a subplot (but the cases are solved exclusively with logic and realism).

The ones that come to mind are: Ron Kamonohashi's Forbidden Deductions, Lord El-Melloi II-sei no jikenbo (fantasy mystery series, part of the Fateverse), and Kusuriya no hitorigoto (I'm also reading the novels).
