Also I'm not really motivated by the "official recognition" thing but I have to admit whenever I get those form letters from Annals ("top reviewer") I think to myself: aww that was such a nice letter

I'm in all the smaller categories! I accept a review if: it's an area I want to contribute, I were to get a personal email, or I have a professional relationship with the EIC.

I also have a soft spot for AIDS Behav and Afr J AIDS Res. And I don't review for AJPH, MDPI/Hindawi/etc

totally varies by journal. epub/online ahead of print further complicates it. some journals (eg Soc Sci Med) do continuous publication. at other journals (eg AIDS Behav) the paper can be an epub for >1 yr until it finally gets released 'in print' (grouped together thematically)
