I covered 1-AA (now FCS) for years when it was a 16-team playoff field. Aside from conference champions (no championship games in FCS), three loss teams were almost automatically bounced.

Georgia Southern won the title 4 times (85, 86, 89, 90) and got bounced with three losses in 1991.
